The Center for
Minimally Invasive Neurosurgery
welcomes you as a patient.
Regular office hours are:
Monday thru Thursday 8:30am - 4:30pm
Fridays 8:30am - 12:30pm
Please bring the items listed below with you to your scheduled office visit.
- Your insurance card/cards.
- If Worker’s Compensation case, then written pre-authorization is required. Please bring the written pre-authorization with you to streamline the paperwork.
- All radiology studies (should be no older than the past 12 months) and pertain to your current medical problem. The actual films must include either, a CT scan, MRI or x-rays.
- Prior medical records, such as consultations from the referring physician, operative reports, EMG and nerve conduction results.
- The name, address, and phone numbers of both the referring physician and primary care physician.
- A current and complete list of all medications you are taking.
- The completed New Patient Packet
- If needed a family member or friend to take notes/help with questions.
